How do I get a certificate of a child with a disability who receives state social assistance (SSA)?
Пconfession The recipient of the SSI is issued by the social protection authority at the place of registration.
Who can apply?
A person who is entitled to state social assistance for a child with a disability.
Where to go?
- or to the social protection authority at the place of registration as a recipient of SSI;
- or to the Administrative Service Center (ASC).
What documents are required?
- an application for issuance of a certificate of the recipient of the State Register of Deeds;
- a copy of the passport of the legal representative;
- a copy of the child's passport (if available);
- a copy of the legal representative's tax identification number (identification code);
- a copy of the child's RNOCP;
- copy medical report about a child with a disability under the age of 18;
- one 3×4 cm color photo of the child;
- one 3×4 cm photo of the legal representative.
Also have it with you:
- originals of the said documents;
- a certificate of registration if the passport is an ID card.
Terms and method of receipt
The certificate is issued on the day of application or no later than 15 business days after the appointment of the State Investigation Service.
If no certificate forms were available at the time of issuance of the State Registration Certificate, you can apply for it later at any time.
The legal representative of the child receives the certificate in person.
If the representative cannot come in person (for example, due to health reasons), it is possible to deliver the certificate to the home after a written request.
Important to know
It is recommended to apply for the certificate after the appointment of the State Specialized Investigator.
You do not have to wait for the first payment. We recommend that you check the assignment of the case code for the child's SSI at the social security authority, which means that the benefit has already been assigned and you can apply for a certificate.

How do I apply for a sick leave to care for a sick child?
If your child is ill, you are entitled to a sick leave. It is issued to the person who actually cares for the child: mom, dad, other legal representative, grandmother, grandfather, or other family member who actually cares for the sick child.
Where to get a sick leave?
- at the outpatient clinic at the child's place of residence/registration
- In a hospital, if the child is being treated in a hospital
The doctor creates a medical report in the electronic system. It is used to create an e-prescription. You will receive an SMS notification.
How many days can a sick leave last?
- for a child under 14 years of age - up to 14 calendar days
- for a child over 14 years of age - up to 3 days (in exceptional cases - up to 7 days)
- In the hospital:
- For a child under 6 years of age - for the entire period of stay in the hospital with the child
- for a child aged 6-14 years - for the period during which, according to the conclusion of the Clinical Commission, the child needs individual care
- for a child under 14 years of age in case of serious illnesses (oncology, AIDS, etc.) - for the entire period of hospitalization
If several children are ill at the same time, one sick leave certificate is issued to care for them. If children fall ill at different times, a sick leave certificate is issued separately for each case.
If another child falls ill during the sick leave or the first child develops a new disease, a new sick leave certificate is issued.
The employer should be notified of the e-sick leave in any convenient way. You do not need to provide printed documents.
You can check the status of the e-medical certificate on website Pension Fund of Ukraine. The Diia portal allows you to check whether a medical certificate of temporary disability has been issued.
For the period of martial law, an e-sick leave can be issued even on the basis of medical documents issued abroad.
How to get free legal support?
Children have the right to free legal aid, including primary and secondary legal aid.
Children with cancer who have reached the age of 14, their legal representatives (parents, guardians, etc.), foster caregivers, as well as children themselves, have the right to apply for free secondary legal aid on issues related to children.
Free primary legal aid
This is:
- providing legal information;
- consultations and clarifications on legal issues;
- drafting applications, complaints and other non-procedural documents;
- assistance in accessing secondary legal aid or mediation.
Who provides:
- executive authorities and local governments;
- centers or specialized institutions providing free legal aid;
- legal entities under private law that have the relevant right;
- lawyers or other professionals engaged by local governments.
How to get it:
- apply in writing or orally to the relevant authority, institution or specialist;
- attend a personal reception;
- submit a written or oral request directly to a free legal aid center.
Free secondary legal aid
This is:
- protection;
- representation in courts, authorities or before other persons;
- drafting procedural documents (lawsuits, court applications, etc.).
How to get it:
Submit a written application in person or by mail to the FLA center at the place of actual residence.
In the application, indicate:
- Name, address, contact information;
- the essence of the issue;
- which privileged category the applicant belongs to.
Attach copies of documents confirming the identity and the right to free legal aid to the application.
Within 10 days, the FLA Center makes a decision and appoints a lawyer or an employee to provide assistance.
