We announce a tender for the supply of medical equipment

The Zaporuka Charitable Foundation announces a tender for the supply of medical equipment. The tender is being held to purchase medical equipment and donate it to charity.

The Foundation will purchase the goods from one supplier selected by the Foundation based on the results of this tender selection within 3 working days from the date of the tender closing. A tripartite agreement will be signed with the selected supplier(s) (Supply agreement.pdf)

The procurement will take place within the framework of the project "Improving Medical Care and Support for War Victims in Ukraine".

General information

Payer: ZAPORUKA Charitable Foundation (hereinafter referred to as the Foundation)

Payment terms for the procurement item: 50% prepayment, 50% post payment (within 3 business days)

Procedure and terms of the tender procedure:

The Tenderers shall be given no more than 7 calendar days from the date of publication of the Announcement to prepare and submit tender proposals for participation in the Tender Procedure.

Deadline for submission: until 07.07.2024

The Tender Proposal shall include documents (copies thereof) that reflect reliable information about the Tenderer, as well as the price, terms, procedure and other essential conditions for the supply of goods/performance of works/services, in particular, but not exclusively:

- A certificate with general information about the company, which containslegal name, legal address, EDRPOU code, e-mail and phone number of the responsible manager, full name of the responsible manager, contacts and full name of the director, bank details, a copy of the license for trade in medical equipment and other medical products;

- Documents confirming the quality of goods/services (certificates of conformity, regulatory and technical documents, a document confirming compliance with the terms of the applicable DSTU and GOST, and/or other documents confirming the quality of the goods and the possibility of their full use for the intended purpose (if necessary);

- Commercial offer (information on the cost of the procurement item, terms of delivery and sale of goods/performance of works/services to be procured by the Fund, warranty obligations for goods and/or results of work/performance of services.

Deadlines for clarification and evaluation of received tender proposals

A potential Tenderer has the right to apply to the Fund within 5 days from the date of publication of the announcement (until July 5, 2024) for clarification on the procedure and/or conditions for submitting a tender proposal.

UPDATED INFORMATION 

Due to the shelling of the Okhmatdyt and the involvement of the Foundation's team in helping the affected children, the consideration of the tender proposals was postponed and scheduled for 08/30/2024 at 10:00 a.m.
